SUMMARY OF POSITION:
This position primarily coordinates services to children and their families involved in Comprehensive Community Services (CCS), along with the Coordinated Services Team (CST) program, Children’s Community Options Program (CCOP) and the Children Long Term Support (CLTS) program with a recovery oriented, person centered team approach. The role of the position is to ensure that the service plan and service delivery to consumers is integrated, coordinated and monitored, to develop service plans, to work with all involved to help empower families with skills and resources they need to address their child’s mental health/AODA concerns, to respond appropriately to crisis situations, and to help families improve their functioning.
ESSENTIAL DUTIES INCLUDE:
Provides Service Facilitation, primarily for Comprehensive Community Services (CCS), along with the Coordinated Services Team (CST), Children’s Community Options Program (CCOP) and Children’s Long Term Support SED Waivers (CLTS)
- Conducts State designed Functional Screen to determine which of the four programs (CCS, CLTS, CCOP and/or CST) that the child is eligible for.
- Provides orientation to the family and service providers who are new to any of the programs.
- Assists the family in developing a team to work collaboratively on behalf of the child and family involved in programing.
- Complete comprehensive assessments for each consumer in order to identify strengths, needs, values, priorities, and recovery goals.
- Schedules and facilitates team meetings to review the assessment results and to develop the recovery oriented service plan for families which will outline each team member’s responsibilities, outcome expectations and outcome measurements
- Monitors the implementation of service plans for progress towards goals and consumer satisfaction
- Coordinates ongoing delivery of services with consumer, family, community agencies and providers.
- Assists in the development of a crisis/safety plan for each child, in which will cover situations at home, in the community, and at school.
- Assists with crisis or emergency situations.
- Evaluates and implements appropriate respite care resources for respective family’s treatment needs.
- Participates in arranging appropriate placements for consumers
- Conduct home and community visits as needed.
- Provides consumer transportation when appropriate.
MINIMUM QUALIFICATIONS
- Bachelor degree in social work or a related human service field required
- Two years experience working primarily with children and families, including programs dealing with mental health or AODA issues, within the last ten years.
- Certification or eligible for certification as a Social Worker in Wisconsin preferred, but not required.
