Description

Do you have a background in social work and the passion to help members of our community? Portage County has a full-time Social Worker position available in the Special Needs Program section available. 
 

SUMMARY OF POSITION: 

This position coordinates services to families involved in Children’s Long Term Support (CLTS) Medical Assistance Waivers for Children.  It is responsible for completing assessments, service plan development, and service coordination for these children and families. This position also collaborates with community agencies and local school districts thorough the provision of case management services.

ESSENTIAL DUTIES INCLUDE:

Service Coordination for Children with Developmental Disabilities, Physical Disabilities and Severe Emotional Disabilities. Duties include;

  • Provides comprehensive needs assessment.
  • Develops individualized service plan.
  • Provides on-going supervision, monitoring, and follow-along to each assigned case.
  • Performs program assessments as necessary.
Assists in the Development, Supervision and Service Provision of the Service Plan.  Duties include;
  • Assists the family in developing a plan for services and resources to meet need objectives and outcomes.
  • Assists in developing a safety plan for each child covering situations at home, in the community, and at school as needed.
  • Supervises the implementation of the service plan
  • Coordinates delivery of services.
  • Provides services when needed such as parent training.
  • Evaluates appropriate respite care resources for respective family’s needs.
  • Conduct home and community visits.
  • Maintains complete and accurate case file to include all required documentation, case        notes, etc.

MINIMUM QUALIFICATIONS

  • Bachelor degree in social work or a related human service field required
  • One year experience working primarily with issues pertaining to Children's Long Term Support Waivers within the last five years required.
  • Certification or eligible for certification as a Social Worker in Wisconsin preferred.
