SUMMARY OF POSITION:
This position primarily provides Birth to Three Program service coordination to children with developmental delays or disabilities and their families including evaluation, assessment, service plan development, and provision of follow-along/service coordination services. This position also collaborates with local school districts, medical providers, childcare providers and other community partners.
ESSENTIAL DUTIES INCLUDE:
Service Coordination for Children with Developmental Delays or Disabilities
- Provides comprehensive needs assessment
- Facilitates and participates in the development, review and evaluation of Individualized Family Service Plans and Transition Plans
- Provides on-going supervision, monitoring, and follow-along to each assigned case
- Coordinates screenings, and evaluations as necessary
- Respond to emergent situations as they arise
- Coordinate with community service providers, such as medical and other health care providers, as necessary
- Acts as a client advocate when the need arises
- Completion of Mandatory record/case reviews
- Thorough and timely documentation of all activities as required by various fund sources. (PPS, Datemart, etc.)
- Participate in a Primary Coach Approach to teaming for the Birth to Three program
- Facilitates access to services and coordinates and monitors the timely provision of services
- Provides information and referral services as necessary
- Ensure compliance with Federal indicators.
MINIMUM QUALIFICATIONS
- Bachelor degree in social work or a related human service field required
- Two years experience working primarily with children with developmental delays or disabilities. Experience within the last five years preferred.
- Certification or eligible for certification as a Social Worker in Wisconsin preferred.
