Description

The Portage County Health & Human Services department is hiring a full-time Social Worker to support the Community Support Program / Comprehensive Community Services section.

Pay range starts at $31.65 / hr

Summary of position:

 

This position participates in a team approach to maintain a comprehensive Community Support Program (CSP)/Comprehensive Community Services (CCS) for consumers with a diagnosis of a mental disorder and/or a substance use disorder.  Its role is to ensure that the service plan and service delivery for each consumer is integrated, coordinated, and monitored and is designed to assist the consumer to achieve the highest possible level of independent functioning  It provides CSP/CCS service facilitation, consultation services to individuals and their families, crisis intervention, and court-related services under Chapter 51 and 55. 

What are the primary job duties?

Provides Service Facilitation Services

  • Complete comprehensive assessments for each Community Support/Comprehensive Community Services consumer in order to identify strengths, needs, values, priorities and recovery goals.
  • Develops recovery-oriented service, individualized service plans for CSP/CCS consumers that address consumer needs and recovery goals identified during the assessment process.
  • Monitors service plans for progress toward goals and consumer satisfaction. 
  • Provides service facilitation and coordination to community agencies for consumers.
  • Facilitates regularly scheduled team meetings for the consumer and his/her recovery team. 
  • Participates in arranging appropriate placements for clients as deemed clinically necessary.7Records clinical treatment notes and progress notes according to program standards.
  • Prepares and presents client cases for clinical supervision.
  • Monitors inpatient treatment plans and develops discharge plans for assigned clients in psychiatric facilities.
  • Facilitates client transportation when appropriate.
  • Conduct home visits as needed.
  • Assists with client crisis or emergency situations. 

Provides Crisis Intervention Services Relative to Chapter 51 and 55 Proceedings.
 
  • Conducts interviews with client, family, law enforcement, medical personnel, and others as appropriate to assess the nature of the problem.
  • Assesses situation and determines appropriateness of referral to Corporation Counsel office for legal action.
  • Screens information and prepares dispositional recommendations to Corporation Counsel office.
  • Initiates court proceedings, arranges witnesses, testifies at hearings, completes court reports and makes recommendations to the court for appropriate treatment and facilities as relates to Chapter 51 and 55.
  • Provides consultative services to family members before and after hearings regarding process, options, and results.
  • Monitors inpatient treatment plans and develops discharge plans for assigned consumers in psychiatric facilities.
  • Provides case management and follow-along services to post-commitment clients.
  • In conjunction with treatment staff, formulates recommendations for dismissal or recommitment to Corporation Counsel.

What are the minimum qualifications?
 
  • Bachelor degree in social work or a related human service field required.
  • One year experience within the last ten years working with persons with a diagnosis of a mental disorder required.
  • Certification or eligible for certification as a Social Worker in Wisconsin preferred.
