This position provides wraparound services for youth who have complex needs and are involved in the child welfare system. Intensive case management services are provided to the youth and their caregivers.  The position serves as the Coordinated Services Team (CST) Service Coordinator as required within Wisconsin Statute 46.56. The position meets with eligible youth and their caregivers regularly and assess the youth’s strengths and needs. The position works with the youth and caregivers to identify client centered goals and implement a plan of care.  

• Meets with the youth and caregivers regularly and provides support on an ongoing basis. Frequent contacts in person or via phone will occur during crises or challenging times. 
• Refer youth and families for appropriate services within the agency or in communities. This includes service referrals for parents to strengthen parental capacities.
• Consult  with service providers working with the family (i.e. therapists; school staff; parent coaches; CCS Worker) to monitor progress and needs. 
• Work with the youth and family to implement a plan of care with measurable goals and monitor progress on an ongoing basis.
• Work with the youth and family to form their Coordinated Service Team. Coordinate and facilitate team meetings regularly.
• Complete case plans and permanency plans as requested.
• Complete required child strengths and needs assessments and enter into the reporting system. Collaborate with other agency staff to meet the youth and family’s needs.
• Completes necessary documentation in the eWISACWIS computer system.

• Engage in family find activities including Seneca searches, and other search engines to find family members. Include youth in family find activities as appropriate.
• Call family members, kin-like supports, treatment foster agencies, adoption agencies, and foster care coordinators as needed to seek out placements.
• Complete child presentations with prospective placement providers.
• Engage in creative recruitment activities to locate placement resources and include the Coordinated Services Team in this effort.
• Keep data of child recruitment strategies utilized for reporting.
• Evaluates substitute care during placement period.
• Complete child recruitments through ewisacwis and complete all placement paperwork necessary.

• Bachelor’s degree in social work or a related human services field required.
• Certification or eligible for certification in the State of Wisconsin as a social worker preferred.
• Experience in child protective services preferred.
• Knowledge of Child Development preferred.
• Experience working in the field of mental health preferred.
• Any combination of education and experience necessary to perform the essential functions of the position.
• Must be a licensed driver and have access to an insured vehicle.
