This is a brief description of the job duties:
 

The Information and Assistance Specialist – RN provides linkage between older individuals and adults who have physical and/or developmental disabilities with the services delivery system and administers the Long Term Care functional screen to assess whether individuals are functionally eligible for publicly funded Long Term Care Programs. Duties include: responding to requests for individualized assistance, scheduling and completing assessments of individuals, providing comprehensive and accurate information about options that are available to individuals and their family members, assisting individuals to gain access to Aging and Disability Resource Center services and those of other providers, developing promotional materials and presentations, conducting prevention and outreach activities, coordinating support programs, entering information from functional assessments into an internet-based program that calculates a level of care; and timely and accurate submission of appropriate paperwork.

These are the skills we are seeking:

Bachelor's degree in social work, psychology, sociology, nursing, or related field.

Current Wisconsin Registered Nurse License required.

One year experience working with older adults or adults with physical and/or developmental disabilities.

Community based care experience is preferred.

Journalism or public relations experience is preferred.
